Outstanding or just ‘good’?

I’m looking at what research has been conducted into quality of care in domiciliary care companies in the UK.

Why is it that fewer than 2% of all companies are rated as ‘Outstanding’ by the Care Quality Commission (CQC)?

In particular, why is it that home-care companies in my part of the world (South East) do not appear to improve?  Anecdotal evidence (albeit from CQC sources) appear to suggest that having gained a poor rating, many companies show no improvement over the next 12 to 24 months.  Those that do improve, only appear to improve enough to satisfy their CQC inspector.

I am in the process of drafting a research proposal on the subject. August 2017
